Indonesia's National Health Insurance (NHI) initiative has demonstrably contributed to the expansion of universal health coverage (UHC). Data from the 2019 nationwide survey conducted by The Ministry of Health of the Republic of Indonesia, specifically the section on 'Abilities and Willingness to Pay, Fee, and Participant Satisfaction in implementing National Health Insurance in Indonesia,' was used in this secondary data analysis. A weighted sample of 18,514 impoverished individuals from Indonesia served as the study population. The dependent variable, NHI membership, was examined in the study. Meanwhile, seven independent variables—wealth, residence, age, gender, education, employment, and marital status—were scrutinized in the study. The findings from the study suggest that NHI enrollment is more common among the poor, particularly among those with higher education, urban residence, being above 17 years of age, being married, and having greater affluence. For the impoverished segment of the population, a higher level of education is a significant predictor of NHI membership, compared with those having lower educational levels. The variables of residence, age, gender, employment, marital status, and financial resources each contributed to their NHI membership prediction. Primary education, in the context of poverty, is associated with a 1454-fold increase in the likelihood of becoming an NHI member, in contrast to those without any formal education (Adjusted Odds Ratio 1454; 95% Confidence Interval 1331-1588). Secondary education attainment is correlated with a 1478-fold increased probability of NHI membership, in contrast to individuals with no formal education (AOR 1478; 95% CI 1309-1668), a notable difference. Pinpointing the patterns and associations of physical activity (PA) and sedentary behavior (SB) is vital for crafting effective lifestyle strategies for children and adolescents. This systematic review (Prospero CRD42018094826) investigated the co-occurrence patterns of physical activity (PA) and sedentary behavior (SB), and their relationship to demographic factors, in boys and girls from 0 to 19 years of age. Five electronic databases were the source of the search. Using the authors' descriptions as a guide, two independent reviewers extracted cluster characteristics. Any disagreements were settled by a third reviewer. Seventeen studies selected for the analysis contained participants between six and eighteen years of age. Nine cluster types were identified in mixed-sex samples; boys showed twelve types and girls, ten. Female groups displayed characteristics of low physical activity and low social behavior, alongside low physical activity and high social behavior; conversely, the majority of male clusters exhibited high physical activity and high social behavior, and high physical activity accompanied by low social behavior. Sociodemographic details demonstrated a paucity of associations with all the identified clusters. In the High PA High SB clusters, boys and girls exhibited elevated BMI and obesity rates across the majority of assessed correlations. Alternatively, the High PA Low SB clusters displayed lower BMIs, waist circumferences, and a smaller proportion of overweight and obese individuals. There were variations in the cluster patterns of PA and SB, dependent on whether the subjects were boys or girls. In both boys and girls, children and adolescents in the High PA Low SB clusters presented a more positive adiposity profile. China's medical system reform prompted Beijing municipal hospitals to explore a new pharmaceutical care model, introducing medication therapy management services (MTMs) in their ambulatory clinics since 2019. In China, our hospital was among the initial medical facilities to establish this service. The reports available concerning the impact of MTMs in China, as of this moment, were relatively few in number. We detail the MTM implementation in our hospital, examine the applicability of pharmacist-led MTM programs in outpatient care, and assess the effects of MTMs on patient medical expenses in this study.
A comprehensive university-affiliated hospital in Beijing, China, was the setting for this retrospective study. To be part of the study, patients had to have complete medical records and pharmaceutical documentation, along with receipt of at least one Medication Therapy Management (MTM) intervention between May 2019 and February 2020. Pharmacists provided pharmaceutical care, aligning with the American Pharmacists Association's MTM standards. This entailed determining the number and classification of medication-related patient concerns, identifying medication-related problems (MRPs), and developing corresponding medication-related action plans (MAPs). Documented were all MRPs identified by pharmacists, along with pharmaceutical interventions and resolution recommendations, while also calculating the cost-reductions treatment drugs could offer to patients.
In ambulatory care, 112 patients received MTMs, of whom 81, with complete records, were part of this study. A significant portion, 679%, of patients presented with five or more concurrent medical conditions, with 83% of this group concurrently using more than five medications. Analysis of Medication Therapy Management (MTM) data from 128 patients revealed that a substantial 1719% of the demands recorded concerned the monitoring and judgment of adverse drug reactions (ADRs). The patient data showed 181 MRPs, and on average, there were 255 MPRs for each individual. Adverse drug events (1712%), nonadherence (38%), and excessive drug treatment (20%) comprised the top three MRPs. Pharmaceutical care (2977%), adjustments to drug treatment plans (2910%), and referrals to the clinical department (2341%) topped the list of MAPs. The multifaceted care needs of residents in nursing homes, coupled with a shortage of nursing staff, present considerable difficulties for healthcare professionals. Hence, nursing homes are undergoing a transformation to become personalized home-like facilities that focus on patient-centred care. Nursing homes' evolving needs and the associated difficulties underscore the importance of an interprofessional learning culture, yet the enabling aspects of its establishment remain largely unknown. This scoping review's methodology targets the identification of those facilitators, focusing on the mechanisms that foster this outcome.
The JBI Manual for Evidence Synthesis (2020) served as the framework for the conducted scoping review. During the years 2020 and 2021, a search was undertaken, encompassing seven international databases: PubMed, Cochrane Library, CINAHL, Medline, Embase, PsycINFO, and Web of Science. Facilitators of an interprofessional learning culture, as reported, were independently extracted from nursing home sources by two researchers. Employing an inductive clustering strategy, the researchers sorted the facilitators into meaningful categories.
After thorough examination, 5747 studies were identified. Thirteen studies, satisfying the inclusion criteria, were incorporated into this scoping review after the removal of duplicates and the screening of titles, abstracts, and full texts. Forty facilitators were categorized into eight distinct groups: (1) a shared language, (2) shared objectives, (3) clear responsibilities and assignments, (4) knowledge acquisition and dissemination, (5) working procedures, (6) supporting and encouraging creativity and change under the leadership of the frontline manager, (7) receptiveness, and (8) a safe, respectful, and transparent setting.
